OK “name”, on your first visit we will collect your MRI, you will fill out your paperwork, and have a consultation with the Doctor’s assistant. That will conclude your 1st visit. Your MRI and file then goes to Dr. Mannella later in the day for him to evaluate. If he determines you are a candidate for care, you will then have your 2nd visit with him. He will explain your entire case, your treatment recommendations, demonstrate the treatment to you, and go over your insurance and out of pocket fees. Normally a New Pt evaluation is $150, but since you heard about us through _______________ it’s only $49 for you. That $49 covers both visits you will have. If the doctor determines you are not a candidate, the $49 will be kindly refunded to you.

CONFIRM POLICY
I would like to confirm, you will not be seeing the doctor on your first visit and you must bring your MRI, CT images. Without them, there is no way for the doctor to evaluate your case.
